In the world of Narutotsunade and katsuyu, many ninjas are capable of summoning animal companions. These summonable animals are often far more intelligent than normal animals and can speak, and many of them assist ninjas in combat or other duties.

Many years later, the Sannin confronted each other using their summons and Katsuyu fought against her comrades. When Pain attacked Konohagakure, the Fifth Hokage summoned Katsuyu to protect and heal everyone in the village, and gave one of the clones to Naruto Uzumaki to receive intel about the Akatsuki. After the entire village was destroyed and Naruto transformed into the Nine Tails, Katsuyu transmitted this information to everyone in the area and Sakura ordered the many slug clones to evacuate the area in fear of more destruction. She was also responsible for informing Naruto that everyone in Konoha was coming back to life. When the Fourth Shinobi World War broke out, Katsuyu was initially in charge of connecting the battlefield and Headquarters, with many of her fragments in different divisions. Later, Katsuyu and Tsunade teleported to the battlefield to fight Madara Uchiha with the rest of Kage. After the battle, Katsuyu's entire body was summoned to heal the Five Kage after their defeat.

Katsuyu resides in the legendary Shikkotsu Forest. Katsuyu is quiet and more subservient to its master especially when compared to Manda or Gamabunta. It addresses Tsunade with the highly respectful honourific "-sama", and, in the English versions, calls her " Mistress " or " Milady ". Katsuyu tends to also use honourifics like "-kun" and "-san" on other villagers. Sakura and Shizune show it considerable respect, addressing it as "Katsuyu-sama". In the English dub, it is referred to as "Lady Katsuyu". It tells Tsunade of the way Sakura overreacted when it fabricated a story of its difficult journey including: the physical limitations of its slug body, falling off a rope bridge, and being attacked by villagers in reaching Team Three during the Three Tails Arc. Tsunade comments on Sakura's naivete and they both share a chuckle at Sakura's expense. Katsuyu is an enormous entity whose true size is unknown. The general size of the summoned portion of Katsuyu is easily as tall as the largest building in Konoha.
