We now know that Captain Unohana is the first Kenpachi, the strongest shinigami. She also earned the moniker "Retsu," which I think means something like "violent," and based on Kiriniji's comments to her, she apparently used to be very violent and killing is where her true talent lies.

Zaraki meets Unohana down there and reveals that they've fought once before, each leaving an aching scar on the other. Zaraki even admits that fighting Unohana , who used to be known by the name Yachiru, was the first time he ever felt fear in combat, a fact that only caused his lust for battle to burn brighter. As they begin to fight, Unohana lands some absolutely fierce blows on Zaraki, even appearing to stab him through the neck at one point. While Kyoraku feared that this training would end with one of them dead, it seems that both Zaraki and Unohana really are fighting as if it's life or death, the only true way to awaken latent potential. Zaraki was already badly injured during the invasion of the Quincy, although it wasn't as severe as Byakuya's wounds. With Head Captain Yamamoto dead and Byakuya in recovery, the Soul Reapers are already down two captains, putting them at a major disadvantage should the Quincy attack again.
This action scene in TYBW Episode 9 is far more than an excuse for another training sequence or to please Kenpachi's many fans.

After the Central 46 fall silent at this, Shunsui observes that this means he has been given permission and calls in the person he intends to have handle this, prompting 4th Division Captain Retsu Unohana to enter the hall while Shunsui identifies her as Yachiru Unohana, the First Kenpachi.
